Boiling water is one of the most widely practised and recommended typhoid prevention methods, especially in areas where safe drinking water and typhoid concerns remain high. While boiled drinking water does eliminate harmful pathogens, is it enough to provide complete protection against typhoid fever?
How typhoid spreads
Typhoid transmission remains one of the most common causes of infection in regions with poor sanitation and unsafe water supplies. Typhoid spreads through the faecal-oral route, meaning the bacteria enter the body when a person ingests contaminated food or water. Typhoid fever can also spread through people who have recovered from the disease, known as asymptomatic carriers, who continue to spread the bacteria without showing symptoms.[1]
Salmonella typhi spreads rapidly in communities without access to safe drinking water. In fact, research shows that people who do not consume treated water are twice as likely to fall sick. People living near open sewage systems, or in areas where domestic water pipelines have been contaminated, are at particularly high risk.[4] Maintaining drinking water safety is therefore essential for reducing community transmission.
How boiling water helps prevent typhoid
It kills harmful bacteria
Water safety begins at home. Boiling is one of the most effective household methods for disinfecting water. When done correctly, it kills all classes of waterborne pathogens, including bacteria that chemical disinfectants may not always remove. A study conducted in semi-urban India found that boiling was associated with a 99% reduction in faecal contamination in household drinking water. The World Health Organization (WHO) recommends bringing water to a rolling boil and then allowing it to cool naturally before consumption, without adding cold water or ice.[3]
It makes drinking water safer
A study conducted across communities in Maharashtra found varying levels of faecal contamination in drinking water sources. Only about 17.5% of the water sources studied met WHO drinking water standards.[3] Another study conducted in Bangladesh found that people who consumed untreated water at home were 12 times more likely to contract typhoid than those who drank boiled water.[2] Together, these findings highlight how boiled water and typhoid protection strategies contribute to reducing infection risk.[2,3]
It helps in daily use
The same principles apply to water used in daily household activities. Using treated water for cooking, washing rice, and cleaning lentils reduces the risk of food contamination.[1] Similarly, another everyday typhoid prevention step is washing fruits and vegetables with treated water before consumption, which lowers exposure risk, especially when produce has been grown using contaminated water.[1,2]
Why boiling water alone is not enough
Contaminated food
Multiple studies found that consuming food or beverages outside the home, from unsafe eating conditions at certain restaurants, street vendors or workplaces, was associated with higher typhoid risk.[1] Eating pre-cut fruit, improperly stored food or meals prepared in places without adequate handwashing facilities further increases the risk, as these conditions support bacterial growth, increasing exposure through food.[2]
Poor hygiene practices
Even households that boil their drinking water remain vulnerable if hygiene practices are inadequate.[1] A common practice in many Indian households is storing drinking water in open, wide-mouthed containers. This can lead to recontamination when people scoop water using unwashed hands or unclean utensils.[2,3] Poor typhoid hygiene practices, particularly failure to wash hands with soap, safe water handling and hygienic storage containers for water, are among the factors most strongly associated with typhoid risk globally.[1]
Environmental exposure
People who maintain good hygiene and food practices may still face exposure to typhoid through their surrounding environment. Open sewage can contaminate domestic water sources, especially during the rainy season, creating a community-level reservoir for Salmonella typhi.[4] Unsafe waste management has also been linked to increased typhoid transmission across multiple countries.[1] While better household hygiene can reduce exposure, it may not eliminate the disease entirely because transmission can have multiple sources of infection, often originating outside the home. This highlights the importance of strong community infrastructure and public health measures in controlling typhoid.[4]
Complete ways to prevent typhoid
Vaccination
Vaccination is one of the most effective individual-level tools to prevent typhoid. A study conducted in urban Bangladesh found that living in a community where children had been vaccinated was associated with a 55% reduction in typhoid risk across the population, including among unvaccinated adults. When vaccination was combined with improved household water and sanitation conditions, typhoid risk fell by 71%. By comparison, following water, sanitatio and hygiene (WASH) practices alone reduced risk by 37%.[4] These findings reinforce the typhoid vaccine’s importance in long-term disease prevention and community-level protection.
Safe food practices
Safe food practices at home and outside are essential. Protective eating habits include washing your hands with soap before eating, reheating cooked food before serving, washing raw fruits and vegetables thoroughly, using clean serving spoons and refrigerating cooked food promptly. Good food handling practices and consistent kitchen cleanliness help reduce contamination risk. Eating food prepared outside the home carries additional risk because consumers have less control over how food handling, storage and hygiene standards.[1]
Hand hygiene
Good hand hygiene is one of the most protective individual behaviours against typhoid. Poor hygiene, especially inadequate handwashing, is strongly associated with increased typhoid risk. Maintaining strong daily hygiene habits like regular handwashing with soap and water, or sanitising when soap is unavailable, lowers the likelihood of infection. Since infected individuals can shed bacteria before symptoms appear, everyone in a household should practise consistent hand hygiene, not only those who are visibly ill.[1] People who do not wash their hands with soap after using the bathroom face a particularly high risk.[2] These simple yet effective measures will help you stay safe from typhoid.
Avoid risky water sources
Contact with surface water sources such as rivers, ponds or unprotected open water has been associated with increased typhoid risk in multiple studies.[1] While travelling or eating outside, avoid drinking water or beverages unless you are certain the water has been treated. Fruit juices and drinks containing ice made from unknown water sources should also be avoided as part of basic health precautions.
When should you be extra careful?
- Eating food from street vendors or roadside stalls increases the risk of exposure, as vendors may have limited access to handwashing facilities and may use untreated water for food preparation.[1,2]
- While travelling, avoid drinking water from untreated sources or consuming food prepared under unhygienic conditions. Eat only freshly prepared, hot food.
- In shared spaces such as workplaces and schools, where toilets and drinking water facilities are commonly used, the risk of typhoid exposure may be higher.[4]
- In all these situations, vaccination, along with consistent water treatment, careful food choices and proper handwashing provide the most reliable protection.[1,3,4]
Conclusion
Boiling water is a meaningful and well-supported step toward reducing typhoid risk because it eliminates harmful bacteria and improves drinking water safety.[3] Yet, evidence also shows that boiling alone cannot prevent every route through which typhoid spreads.[1,2,3] Food contamination, inconsistent hygiene practices and recontamination during storage can all continue to expose people to infection.[1,2,3,4] The most effective and common prevention methods against typhoid are vaccination, combined with consuming treated water, safe food practices and regular hand hygiene.
